I have done this before but last time it was all taken care of by my vet (he got the form, filled it out and gave it to me).

When leaving Korea, one must get an official form the National Veteranary (sp??) Association proving that the dog got their Rabies shot.

Two question:
1) Does anyone know if that form is availible on the internet to download
2) Where are the NVA buildings; specifically in busan?
