Put these clauses in the appropriate places:
According to the pertinent laws of the Republic of Korea, all applicable taxes, including, but not limited to, the Korean income tax and pension contributions, shall be withheld each month from Employee�s salary. Employer is to pay these amounts to the proper government authorities and to provide employee with a receipt monthly evidencing that the government has received these monies.
Income Tax deduction shall not exceed 3.3% of Employee�s monthly salary.
Pension contributions shall not exceed 4.5% of Employees monthly salary and with a like and equal amount to be paid by Employer.
Employer shall provide Employee with medical insurance through NHIC. The cost/premium should be 50/50, with no more than 2.24% being deducted from each paycheck. Fifty (50) percent of medical insurance premiums and fees shall be paid by Employee, and fifty (50) percent shall be paid by Employer. Employer agrees to provide Employee with a medical insurance card issued to the Employee from the National Health Insurance Corporation within 30 days of the Employee's entry into Korea.
Employer agrees to provide Employee with an Alien Registration Card within 20 calendar days of the Employee's entry into Korea.
Housing provided by the Employer will be furnished housing to include, but not limited to, a bed, washing machine, telephone, air-conditioner, refrigerator, stove, chair, table, television, cooking utensils, eating utensils, blanket, and clean linen. All items when delivered to Employee to be clean and in good working condition and order.
The Employee shall take care of the furniture and equipment, and shall be liable for the cost of repairs or replacement to any equipment or furniture damaged as a result of negligence but where used furnishings are provided, instructor shall not be responsible for replacement of said furnishings and equipment due to wear and tear from normal use.
Employer shall provide the Employee one-way economy-class airfare for the most direct route from the city where the Korean Embassy/Consulate to which the Employee submitted his/her VISA application is located and Seoul-Incheon International Airport. Employer agrees to purchase the one-way airfare for the Employee prior to Employee departing for Korea.
Employee shall be entitled to a paid leave for a total of two working weeks (to mean 10 working days) during the Period of the Employment. Annual leave days shall exclude Saturdays, Sundays, and Korean National Holidays. The Employee is entitled to five (5) days of consecutive annual leave in the summer and five (5) days of consecutive annual leave days in the winter. The Employee shall observe Korean National Holidays throughout the contract term. No deductions in pay shall occur from the Employees monthly salary during Korean National Holidays.
