I'm trying to implement a sharing relationship with my co-teacher, and I need some help about lesson observations/idea sharing.

I found this observation form in my TEFL coursebook:

I want people to add to this form, as I feel it's not perfect, but at least a good starting point. It doesn't feel sensitive enough to me. I can't place my feeling, but when I look at this sheet, I keep wondering about all the other things I can see during a class, but not have the space to fill in.

What are the best additions to make, yet still keep the form streamlined and efficient?

What would you change/add?

How would you restructure/change it?

Also, not to kill of the thread before it starts, but also am interested in what other structured approaches you use for sharing/communicating.
