From the Korean Immigration English homepage (http://www.immigration.go.kr/HP/IMM80/index.do):- Click "On-Line Service" in the menu bar at the top of the page, then "Forms"
- Choose "Reference," #18 on the page that appears
- Download the form in the appropriate format (probably PDF for non-Koreans)
You shouldn't have to deal with this, though. This form is submitted to Korean Immigration in Korea by your employer. After the visa is processed and approved in Korea, your employer will provide you with a Visa Issuance Number. You submit that number, along with a completed Application for Visa (http://www.immigration.go.kr/HP/IMM/imm_forms/image/visa_2.pdf), passport photos, and your passport to the Consulate, who will issue the visa and place it in your passport. |

As stated elsewhere, we probably do not have to deal with that form. I e-mailed the Korean Consulate in SF to ask what I need to bring and this was her reply:
1. After you receive approved documents, you can submit Contract, Approved Document, Copy of Diploma, an Application for Visa, One photo, $45(Cash or Money order), American Passport.
2. You do not need to bring copy of criminal record check.
If you have questions, their e-mail address is on the website's home page. They responded in about one day. |
