While the choices are natural for some answers, the two sentences are not natural sentences at all.
How have you been doing recently? The recently is a little too formal. You could leave it off, or use lately.
The answer could be A, B, or C, but not D.
Nothing really goes wrong. Very unnatural. Maybe: Not bad. Everything's doing fine.
