They all make sense, with various meanings.
The word "relationship" basically means a connection. It can be used for a family connection, but recently the expression "in a relationship with..." has come to mean a romantic relationship. Thus the last sentence, 4,means
Is she your girlfriend?
Numbers 1 and 2 might be referring to this, or they might be asking about a family connection, or even if you know the person well, is he or she your boss, etc.
Number 3 means, why are you asking me this question - what importance does it have for you. It is a little bit rude.
