Simple courtesy says that the caller should identify himself / herself first:
Answerer: Hello.
Caller: Hello, this is Joe Brown. May I speak with Mary Smith?
A: She's not in right now. Can I take a message?
C: Yes, please. Would you ask her to call me when she gets a chance? It's Joe Brown at [number].
A: I'll give her the message.
C: Thank you. Goodbye.
A: You're welcome. Goodbye.
When old Joe doesn't say who he is, then it's best to ask:
A: Hello.
C: Is Mary there?
A: Who's calling?
C: Joe Brown.
A: She's not in right now. Can I take a message?
[Etc.]
If old Joe won't identify himself, it's best to hang up without telling him anything:
A: Hello.
C: Is Mary there?
A: Who's calling?
C: None of your business.
A: Goodbye. [Hang up.]
