"notice" and "realize" are largely interchangeable, but not always. In this example, I would favor using "realize": with the drugs, you would certainly notice their effects shortly after using them, but on the other hand, the more serious health consequences might not be noticeable except after a period of time. "realize" carries more of this idea of gradualness.
