i would appreciate it if you call back this afternoon for the doctor's appointment.  |

It means, "Please call me this afternoon to discuss the doctor's appointment."
It is a little ambiguous, though. Is it a message from the doctor's secretary to the patient? Is it a message to the doctor for his / her appointment with the caller (perhaps an accountant)?
