Those three sentences are options in ID register. Not sure what does 'taken' and 'open' mean here.

'taken' means I have had a girlfriend/boyfriend, or I am married or I am engaged?

'open' means I am looking for a girlfriend /boyfirend?

3...I am not sure what is meant by "open" here but my guess is that it means 'available', which means 'not in a serious relationship'.
If I say that 'I am open to a relationship.', it means that I am available and am not trying to avoid a relationship, but it doesn't mean that I am actually looking for a boyfriend or girlfriend. If you are looking for a bf or gf, say 'I am looking for a...........'.

Your question grammar is not correct. Say 'Does (taken)(open) mean I ........?' You have to start a question with a question word.

i don't understand what the questions that you provided has anything to do with your personal identification on an application...that's personal information. it this for an ID card, like a driver's license? i'm just curious.
