Do they all mean the same thing?

In Britain if you are whacked, or whacked out, you are exhausted.

If someone is whacko or whacky it means they are not in their right mind, slightly unbalanced or very slightly insane. We would use it to describe eccentricity.

Whacko is also used in the south of England as an informal greeting, similar to What ho! or how are you.

"Whacko mate. How's yer father?"

All of the examples before are correct too. Where I'm from, Georgia in the US, if you say "That guy is whacked out" it can mean that he's high on some drug. Also, let's say your roommate eats the last piece of homemade apple pie your mom made. You can say "Man, I can't believe you ate my pie! That's so whack!" Meaning, it's a really wrong thing to do and disrespecful.
