Which is correct for the blank below, #1, #2, or #3?

There were four survivors ( ) the airplane crash that occurred last summer.

Hi, FW.

The preposition most often used with survivor is of: "There were four survivors of the airplane crash that occurred last summer."

3.) is correct
Airplane is not correct English spelling.

I agree that 1 is correct, and Airplane IS Correct english spelling...

Aeroplane is the standard form in British English. Airplane is the normal form in American English, and it is sometimes found in British English also.
