B:Really? what (is he studying/does he study)?

I think you would hear, "What's he studying?" more.

a more common way to phrase it would be "what is he Majoring in?"

Both replys,

B:Really? what is he studying.
and
B:Really? what does he study.

are grammatically correct. Whichever one you hear more that another one, is only incidental.
