.
Both words have several meanings:
Recognize:
verb: detect with the senses
verb: show approval or appreciation of
verb: exhibit recognition for (an antigen or a substrate)
verb: perceive to be the same
verb: be fully aware or cognizant of
verb: grant credentials to
verb: accept (someone) to be what is claimed or accept his power and authority
verb: express obligation, thanks, or gratitude for
verb: express greetings upon meeting someone.
Realize:
verb: convert into cash; of goods and property
verb: make real or concrete; give reality or substance to
verb: expand or complete (a thorough-based part in a piece of baroque music) by supplying the harmonies indicated in the figured bass
verb: perceive (an idea or situation) mentally
verb: be fully aware or cognizant of
verb: earn on some commercial or business transaction; earn as salary or wages.
Where they are used with the same meaning, they are-- of course-- synonymous:
Recognize: be fully aware or cognizant of
Realize: be fully aware or cognizant of

.
You can say 'I hardly recognize you'; 'I didn't recognize your voice'.
