According to my book, the above sentence means 'Though a man may be poor, do not despise him.'
Is that right? If so, I wonder why and if native speakers use the above quoted sentence now?

Yes, that is what it means. No matter how poor a man is, he doesn't deserve to be despised. (Poor or not, he's still a man.)

I don't think "Let a man be ever so poor" is used much today. But a lot of people understand what is meant. It would be more common to hear something like

"Even though a man is very poor..."

"No matter how poor a man is..."
