Hi

Letters of recommendation are usually actual letters, not forms. They say how you know the person you're recommending, what they did at the company, what they were like as an employee etc. You can use a basic structure for each letter though - here are a few links to some guides:

http://resume.monster.com/articles/recommendation/
http://jobsearchtech.about.com/od/letters/l/aa021400_3.htm
http://www.writinghelp-central.com/recommendation-letter.html

(You can find more if you search for "letter of recommendation" on google)

Is this any help?
