(1) "Do you agree to the contract?" is all right. A contract is, in fact, an agreement, but people do say it this way.
(2) "I'm going to visit to Italy" would be correct if you eliminated the second "to." "This is my third visit to Italy" is fine.
(3) "He is involved in the accident" is all right, although you would probably say, "He was involved in the accident," since it must have happened in the past.
"He is involved with the accident" wouldn't work. To be involved with someone means to have some sort of romantic relationship with him or her. To be involved with something is to have something to do with it.
