Could you tell me what she was doing. I cannot see the picture.

A screen door is a frame covered with insect proof netting. It is the same size as the main door to the house, and is mounted ouside the main door. Its function is to keep out insects when you open the door in summer.
The sentence means that the woman leaned out, i.e. pushed the screen door open with her shoulder so that her head and shoulders were outside the door. At the same time she supported the box she was carrying on her hip.
Her feet would most likely remain inside the door.
