As with most song lyrics and poetry, the meaning may not be just one thing, may not be easy to discern, may be open to interpretation.
For me, the image of a jagged little pill is something that you have to take, have to swallow, but it is not smooth and easy to swallow because it is jagged, and it may hurt--even if it is good for you.
It recalls such things as getting a dose of one's own medicine; having a bitter pill to swallow; taking one's medicine (punishment, what one deserves); swallowing what someone says is true, or saying it is hard to swallow.
