I'm writing a report to report what our team (team xxx)did in last month, namely August, could you advise which title is the most commonly used. thank you

e.g.
1. Team xxx report in August
2. Team xxx report of August
3. Team xxx report on August
4. Report of Team xxx in August
5. Report on Team xxx in August

Hello Philo,

I'm on a team to introduce six sigma concept to our company, we need to set up softwares, give people presentations and train people from each department, and report the progress on a monthly basis. (Six sigma is more like an improvement tool which is very popular among many big companies e.g. GE, Motoralo,etc.)
