It has happened that when I went to his place, he didn't let me in.

Does this mean:
a-Sometimes when I went to his place, he didn't let me in.
b-Every time when I went to his place, he didn't let me in.
c-I went to his place once and he didn't let me in.
d-We can't tell. The meaning could be a, b or c depending on the context.

It means that, at least once in the past, you went to his place and he refused to let you in.
