Number 3 is the only correct example. 'Sign' in this context means to (formally) agree, in this case with your signature.
'Sign up' means to join some sort of organisation. Example "I will sign up to the Madonna fan club" or "I will sign up to become a member of a political party"
'Sign on' means to register in some way. Example "You must enter your password to sign on (or in) to the website" or "I need a new job, I will sign on with an employment agency".
In some situations either 'sign on' and 'sign up' can be used.
