What does it mean?
Does it mean that she was pregnent yesterday or she gave birth to a baby yesterday?

It means that she gave birth yesterday. If you say she's having a baby, that can mean she is pregnant and is going to have a baby sometime in the future.

As I reread your question, I could answer it by saying it means both. Yesterday she was pregnant, then she gave birth and was no longer pregnant.
