Which is the correct choice among the four??
I'm almost sure it's "over", right?
If so, what does it mean?
Does that mean she is from a city which is on the other side of the River??

She is from a small city (at, on, in, over) the Tone River.

I very seriously doubt it. The obvious choice to me is on. That means the town she is from is located somewhere along the Tone River. Makes perfect sense to me. We say this every day. All the others sound odd.

About the only possible way over even comes close to making any sense is if the speaker happens to be directly across the river from the town in question at the moment of speaking...and even then I would say across, not over.
