what is the differences betwen "be made of" , "be made from", and "be made by"?

Made of and made from refer to the materials used to make the article. Shoes are made of leather. Cheese is made from milk.
Made by refers to the person or thing that makes the article. Horseshoes are made by blacksmiths.
