1. To every appearance Bob is rich.
2. To any appearance Bob is rich.
3. To any appearances Bob is rich.
4. To all appearance Bob is rich.
5. To all appearances Bob is rich.
6. To all the appearances Bob is rich.

No. 5 is right. The rest are not.

It is a set phrase.
