It probably should read vest and badge, unless each person picks up more than one.

not lips? a tear only?

Why not? You have two lips. The writer says only one was quivering, and only one tear was rolling.
I agree with you it may be unlikely, but that's what the writer wrote.
Grammatically it's OK.
