I would say that both of them are useful. Maybe mixture of the two would be a good idea. And it also depends on the text because if it is a text about a topic your students have no information about, it would be pointless to try to apply their background knowledge.

I think that the best thing to do is asking them to read something they are really interested in. For example books chosen by them, or they can search the internet to find articles about things they are really interested in (sports, fashion, etc.). Maybe, they can then comment on what they have read...

I have never thought about it. But maybe it's because it's a set expression and there is some kind of elipsis there ("that had" has been omitted because you can infer it) But I'm not quite sure about it.
