Hi,
I'm interviewing for an ESL teaching program (similar to Teach for America) in which I am presenting a 5 minute lesson as part of the interview process. I have a lot of ideas, but am having a ton of difficulty creating a fun, interactive, complete & simple lesson in 5 minutes. Does anyone have any ideas?
I'm gearing the lesson toward Middle school students, either beginning or intermediate.

I think the problem is you're trying to plan a lesson, perhaps use ice-breakers, lead-ins or warmers. Many of them can be done in five minutes.
There's lots of materials on the web from many different sources, but if you want to look at a couple of examples that are tightly timed look at the lead-ins in my old CELTA lesson plans. You can download them at:
