Short self evaluation questionnaire

Hi
I am working on a project about self evaluation for the Trinity TESOL Diploma. I need to collect as many replies as possible to a very short questionnaire (see link below with 8 questions). I would be very grateful if any experienced teachers could spare a couple of minutes to answer my questions.
