Issues in teaching reading comprehension

I'm a TESOL MA student researching reading pedagogies/ issues in ESL reading comprehension. What--as teachers--are some things that you encounter? How do you teach reading so that students get the maximum comprehension from readings? What's your approach to teaching reading?
