Transformational Rules - Help

This is strange stuff. I understand the part about surface and underlying structures in sentences like "She hit the man with the bat" meaning she used a bat or the man holding the bat.

But what about a short sentence like - "Where did Bush live?"

Is the underlying sentece - Bush did live where?

And how do I go from that back to the surface sentence?

The reason why this is difficult is because my mind wont get past the fact that I see sentences like "Who Canada did beat in the hockey game?" and they call it Auxiliary insertion and there is no * to call it ungrammatical and I just can't see that sentence being used anywhere at anytime especially as a question.

Maybe someone can help me with this one.
