Student Survey and Self-Assessment

The language school i teach in gives students a survey to fill in at the end of their 13 week course - it ask questions about cleanliness, friendliness, handouts and whether there are any aspects of the school they would like to see changed or add something in to the programme - now they want to add a bit on the back whereby a student self-assesses their own performance in the class - can anyone advise or give me some ideas of what questions could/should be put on the form - about 4/6 questions would be enough - scoring 1-5 - with 5 being the highest and 1 the lowest - your input will help me immensely - thank you
