Report card for beginning English learners

I have taken on the task of developing a report card or report form for K-8 teachers, both ESL and classroom, to use as a supplement to or replacement for our district report card. I am an elementary ESL teacher in public school in Arizona. When classroom techers get s beginner in their classes, they always ask me, "What about the report card?" If you have any such document in you school or distict I would love to see it. My own product will be aligned with the AZ state standards for English Language learners. I just need some format ideas. This is a project for my Career Ladder pay this year. so $ is attached to completion of this project by June 7, 2005.

Do you have categories in each subject for things like effort, participation, behovior, breaking down the skills needed for the subject? That's what we do at the school in WI that I work at. For reading, there's reads well orally, uses phonetic skills, understands what is being read. Language has a seperate item for puncuation. There's also a section for work and study habits. You can customize the list for whatever your needs are.
