Anyone know any good graded (levelled) readers?

I'm looking for a good book series to help my students with their reading. I need a good series that focuses on phonetical sounds and has a low word count at the beginning, but increases vocabulary after the initial phonetic level. I may need to use 2 sets... but can anyone help me?

For young readers, the Ginn series was not bad.
(Don't know if it's still in print)
It seems there are a ton of new series out there, but most aren't much good in elementary. Try Oxford Univ Press's elt page.

In English primary schools, The Oxford reading Tree series is very popular - I'd imagine it could be used just as well with ESL children. They're really nice little books - great stories, lots of humour, fun, multi-ethnic characters with lots of repetition of language.
