I have to prepare a pre-course questionnaire for attendees of a ESL technical writing course.

Are there any resources available to help me prepare such a questionnaire?

Is this a needs assessment (to determine what skills are most needed, and for what type of communication) or a diagnostic test to determine what the students' areas of strength and weakness are?

This is more like a needs assesment.
