Could I please receive some advice on, and insight into, obtaining the "Physical Examination Record For Foreigner" in Canada?
I was living in South Korea at the time I was offered my job in China (June, 2013), and they required that I go to a hospital there and get a physical health check completed. The results were written all in Korea; I had it translated into English and everything was fine when I scanned it and emailed it off.
Now I am expecting to receive my invitation letter, confirmation letter, etc, in the next little while, and the Chinese Visa Application Service Center in Toronto's website is saying that in addition to the documents provided by the immigration bureau in China, I also need a "Physical Examination Record For Foreigner."
So, if there are any Canadians out there who went through this process, is there a simple way of doing this? Or, can I simply use the same health check that I had done in Korea? I still have the original copy of that health check, which was apparently acceptable to the Chinese immigration bureau.
Any advice and/or instructions would be greatly appreciated. I was taken aback when I saw this stipulation listed on the website and now I'm scrambling to figure out what to do because I don't want all those invitation and confirmation documents to come in and me not be ready to apply for the visa immediately.

Is it filled out on the proper form (that's in Chinese and English) or is it entirely in Korean and translated into English? Or something else?
If it's the standard form they want, with your stamped picture, it probably won't be much of a problem. But your best bet is to call the Chinese consulate in Toronto and ask, (416) 964-7260. |

toddt, just to be clear: you didn't use the "Physical Examination Record For Foreigner" (embassy form) and have it filled out for your examination in Korea, and instead, the Korean hospital filled out there own forms only, which you had translated into Chinese? |

Unfortunately, it is a form filled out entirely in Korean by the Korean hospital. I sat down with a Korean speaker and we translated what it said into English, not Chinese. The translation is not official, but it is accurate.
Let's just say that the Korean health check is in no way exactly similar to what the Chinese Visa Application requires, or is expecting; it is, however, a legit medical check (stamped and everything) that tested for the usual things, such as blood, liver, vision, hearing, etc.

It's acceptable for the health check to be provided to the embassy for the Z visa to be completed in English. It doesn't have to be in Chinese.
Whether the specific embassy you are applying to will accept a foreign medical check is up to them i.e anyone's guess.
In my case my Korean medical check was *not* accepted by the Chinese Embassy in Bangkok and I was required to do it again at a local hospital. Only the particular embassy or consulate you are applying to can answer for certain as to what they will or won't accept. Worth making a phone call or two to confirm. |

Ah - Just read this. I highly doubt the embassy will accept a health check that wasn't done using the official form. Chinese bureaucracy is a b1tch. As I said, confirm with the embassy, but if I were you I'd be preparing myself to do another check. |
