I{m writting evaluation for my students to fill out at the end of the semester.
So far I have
What three activities did you like best?
What three activites didin{t you like?
What would you change about the course?

Any other ideas?

Here are some questions my university asks on the end of the semester evaluations:

1. About how many hours a week did you study for this class?

2. The workload for this course is: easy medium difficult

3. What did you learn best from? lectures, handouts, videos, etc.

4. Reason why I took this course: gen ed requirement, major requirement, elective

5. How challenging did you find the course? none medium very

The evaluations had about 20 questions, but I'm having a hard time remembering the other questions. Hope this helps.

We have to fill out pages and pages of forms at the end of our classes at my university. They ask us questions like:

What were the instructors strengths and weaknesses?

Would you recommend this course to a friend? Why or why not?

How often did you attend this course? Circle one. 0-20% 21-40% 41-60% 61-80% 81-100% (with the implication being that people who attended the course were better qualified to judge the class and its teacher; I always felt bad when I had to circle 0-20%... usually I never went because the prof was so horrible!)

And then there are all the strongly agree - agree - neither agree nor disagree - disagree - strongly disagree questions like:

The workload for this course was manageable.
Assignments were graded fairly.
Assignments were returned in a timely fashion.
The instructor made grading standards clear at the beginning of the course.
The instructor encouraged participation.
The instructor treated students fairly and with respect.

etc etc etc

Depending on the level of the students, you might want to give them multiple choice answers instead of open-ended ones.

Here are some of the questions I recall being asked:

What do you expect your final grade will be?

Did you come to class prepared?

How many times have you missed class?

Was the instructor's use of technology satisfactory?

On a scale of 1 - 5, 5 being best, how would you rate the text book?
