Does every EFL position in Saudi Arabia require a medical check to be conducted BEFORE entering the country or before obtaining the work visa?

There are 2 medicals.

One is for the employer. Some companies do not require this.
The other is for the visa. Requirements vary, depending on where you are getting the visa.

You will probably have to have another medical after arrival.

So far as I know, everyone has to do a medical test prior to departure as part of the visa application process. As Scot has said, exact details depend on the Embassy involved.

Regarding a post-arrival medical test, again as Scot has said, this depends on the employer. My previous employers required me to do one, my current employers did not. It is ridiculous that people have to submit to a second medical exam, having just done a very thorough one to get the visa in the first place, but that's another story.
