Medical care is pretty good all over Oman and I am sure that you can have your cast taken care of in Nizwa - however, most basic medical coverage provided by employers will not cover pre-existing or old injuries - you may have to pay for that expense yourself and it is worth asking about.

Well, I don't know exactly but it is likely much cheaper in Oman. You could go down to the clinic, have the cast buzzed off for probably about 20-30 bucks - or even less ...?
