Enrollment in National Health Insurance is mandatory for foreigners residing in Japan for a year or more. If your employer hasn't enrolled you in such a scheme, you can go down to your town, ward, or city office and enroll. |

Yeah, I did it...we will have to back pay for the time we didnt have it, but it is not so much.
But is that the cheapest route? |

In terms of monthly payments, no. Lots of people never enroll in a public scheme and make monthly payments that are as low as ¥4,000.
As far as coverages go, it varies from provider to provider.
I rarely ever go to the doctor (I certainly won't go if I have just a common cold) so I can understand why so many people ignore the mandatory public health insurance requirement. But, it's nice to have when you do need to visit the doctor or make an emergency visit to the hospital. You just show your card, fill out a form, and 70% of the costs are taken care of - you only pay 30% |

I'm on National Health and pay about 1500円 ($15) per month. I live in Tokyo. I see a doctor monthly as I have a chronic condition and the visit is usually under 10000円, including all the medical supplies I need. In comparison, in the US, I was paying about $500/month with insurance and cost of supplies, and saw the doctor once every 4 months because the tests were also expensive. |

1500 yen/month? Are you sure it isn't more like 2500?
And, you are on your first year of making such payments, aren't you? They will go up tenfold or so after that. |
