For the card it says these things need to be provided on the pdf file:

You are required to provide the following :
1) Your written request.
2) Payment for the $18 processing fee, either by certified check or money order made payable to the U.S. Treasury, or a credit card. For credit card payment information, visit the Identification Record Request website.
3) A current, original tenprint fingerprint submission (not previously processed) taken by a local law enforcement agency and bearing your name, date of birth, and place of birth. Key information needed is magnified below.

Also this website says to put a note in saying you need the FBI certificated authenticated so I can get an apostille from the state department.

I have already gotten my fingerprints done at a local law enforcement agency. But what do they mean by provide 'your written request'? Just the cover letter above saying you want your record along with this sheet?
On the fingerprint sheet there are tons of boxes I either don't have info or don't understand. I don't have an FBI number or armed services number. Plus I don't know what 'your number' and 'misc number' are supposed to be about. I get the impression from the FBI that name, date of birth, place of birth, physical descriptions and soc. security number are the important ones. I don't know what the box 'ORI' is about. For 'citizenship' do I just put 'USA' or do I put my state of residence?
The reason I'm cautious is it is my understanding it can take 2 months or more to process this, so I don't want to leave some box blank by accident and end up having to wait another few months to straighten it out. |

Juche, since your questions are so specific, I suggest you call the FBI division that handles fingerprint orders. That's better than wasting precious time waiting for answers on this forum and taking a chance that the responses you'd get here are accurate. |

Yes, just the cover lettter.
In the boxes, if you don't understand, don't fill it out. With no FBI number and no armed service number, you'll still get your CBC.
Yep, name, date, etc, are important. USA is citizenship, state of residence isn't citizenship.
Leave ORI blank.
Mine (all three) have taken 4 months start to finish. |
