I received my visa issuance number and am going to the Korean Consulate in Los Angeles. Is this the right form I need to fill out to give to the Consulate to get my passport stamped:

So if I have this correctly, I take my visa issuance #, my passport, a picture, and the above application to the Korean Consulate?

Hardly matters if it's the right form or not as you can fill out the visa application form at the consulate in about 5 mins, however, don't forget the visa application fee! |

Thanks for the info. We went to the consulate in Los Angeles and that was indeed the same form we filled out there.
Also, the fee was $45.
By the way, anyone ever use boxes instead of luggage to come over to Korea? |
