I received my visa issuance number and am going to the Korean Consulate in Los Angeles. Is this the right form I need to fill out to give to the Consulate to get my passport stamped:

So if I have this correctly, I take my visa issuance #, my passport, a picture, and the above application to the Korean Consulate?

| Hardly matters if it's the right form or not as you can fill out the visa application form at the consulate in about 5 mins, however, don't forget the visa application fee! |

Thanks for the info. We went to the consulate in Los Angeles and that was indeed the same form we filled out there.
Also, the fee was $45.
By the way, anyone ever use boxes instead of luggage to come over to Korea? |
