| Just take it with you to the consulate, along with the application, your passport, 2 photos and the fee. |

At the top of the application page for E2 visa, it says specifically where to put your number. You can download the application form from the immigration site or any embassy site, or pick it up at the embassy.

You can get it from this site. Go to download forms. |
