Seriously...it looks like a joke. I had a physical done last month and tried to give this to the lady at the reception desk and she gave me a "wtf" look. I had to schedule another appointment because the doctor never checked my eyes or have any records of my immunization. *sighs* I hate having to take another day off and spend unnecessary money to have my doctor fill out this "form." I mean...what is he supposed to fill out for "liver, gastrointestinal, cardiovascular, genito-urinary, central nervous system, skin"
Now I have my immunization record...so I'm just going to attach it the medical form. This is just a pain...I also don't have 20/20 vision. My glasses are very, very old. I don't even have 2/20 with them on..
Has anyone actually had this filled out by their doc without any problems? I'm just very annoyed right now. The form looks unprofessional..and it's not specific as to what it is they are looking for. |

When I was applying I told the recruiter that if I was not sick the
schedular would not bump a sick person to put me on the
schedule.
I didn't get a form so I also told her that there was
no way a doctor would know what kind of exam they wanted.
She finally told me I could get it done here. I've never had one.
Other people have said that after paying for one in the states
they still had to get one from a Korean doctor. |

My (Korean) doctor filled out mine without even looking at me. For liver, gastrointestinal, cardiovascular, genito-urinary, central nervous system, and skin, he simply put "no known problem". If your doctor speaks some English and is familiar with at least some of your medical histoty, it shouldn't take more than 5 minutes. Or cost more than 3000 won. |

I am American...and so is my doctor..and he's a new doctor. My previous physicians are either retired or deceased. If they are going to make me take another physical in Korea then I'd rather skip the whole ordeal here. |

When I applied for my public school, all I had to fill out was a WAIVER saying I was healthy, and upon reaching the school, I would get checked out by a doctor. Signed the waiver, and haven't ever been asked/talked to/etc about the check up.
So I don't think you really need to jump through hoops before you get to Korea. I went through ATC recruiting, and there is a copy of the self-medical waiver/form somewhere in the bowels of their website. Do a search, I'm enjoying some pubby school milk and homemade cookies right now... |

The checkup takes 30 minutes and is cheap. I don't see the problem in doing it. |

In America..it is not. I'd have to take a whole day off at work as well. I'd rather do it Korea if I can. |

I have 20/20 vision. I just did a medical CU for my P-school and wasn't even informed of the results...after my blood test and chest x-ray the technician said I could go...this is after I just had one done in LOS for the job around 7 months ago.... |
