Anyone know of a reasonably priced (cheap) and quick service for translating scans of Korean forms into English?

Any pointers or shoves in the right direction appreciated.

*Apologies if this is the wrong forum for such a request.

Failing a translation service, would anyone be willing to translate this:

I've got the owner, name, species, age, sex etc. stuff, it's the bits in the main body and at the bottom I need to ensure I understand.

Thanks in advance.

edited to remove link and to thank the person who's offered translation.

Tony, I've heard it's a 'use it or lose it' thing, so it's prolly just as well.

It seems your dog has been vaccinated until Oct 16.
The rest on bottom is the vet location and business reg.
Is that what you want?

Yup, pretty much. I knew that he'd been vaccinated (of course) but was wondering what the wording was since there seems to be a lot of stuff (vaccine manufacture date?) there just to confirm that he's been given a combined distemper, hepatitis, leptospiriosis, parainfluenza and parvo shot.

Thanks for confirming the date to which he's effectively vaccinated; I appreciate that.

Why not ask a vet or someone at the local pet shop?

Does the layperson know these, or will a vet? There are some letters like DDSP or somesuch. I think it's obvious that only a person in that particular occupation would know it. It's not written anywhere about polio but there is one vaccination called '.......' written in English on the bottom. It is already translated. But what does it mean but to a vet?

Jeez Wayne.

Cheon,
Thanks for your input. I know the vaccines the DHPPL stands for (mentioned in my post above). The vet doesn't speak/write English; I've got the gist of the forms but wanted to have as full a translation as poss just in case there are questions.

Someone's offered to translate them for me (thanks very) so it's all good.

If you post your form and the translation it might help other people.

Dogshed, I'm not sure how many people will be moving critters around but have gotten permission from the person to post the translation in case others need something similar.
