Here's my situation:

I'm currently here in Korea and have an E-2 visa that expires on January 31st. I'm applying for a new E-2 visa, not a renewal at that point (the job is already lined up, and I want to start as close to the beginning of February as possible). I've pretty much got most of the requirements taken care of, I've already got a local U.S. police check and the FBI check is being processed. Also my university degree is in the process of being apostille certified right now. My primary concern is the medical check.

I just had the mandatory Korean physical last week, which I assume checked everything that Korea wants to know about my health. Can I get an official copy of this Korean medical check and use it to fulfill the health check requirements, or alternatively, can I get a second check done here in Korea within the month of January before my current E-2 visa expires to use for the application. This is particularly important for me because I'm taking a December vacation back to the United States. If a U.S. physical is required, this is my perfect opportunity. Unfortunately there's no word on exactly what I need tested nor an official form to give my doctor. I've already made an appointment with my doctor in the U.S. for December 5th, but I'm afraid I'll have to cancel it because I won't know what is needed. Any information I can get about the medical requirements would be greatly appreciated.

For EPIK and the public schools.....

basic blood pressure
heart rate
chest X-Ray
HIV Test
Hep B
Hep C
CBC
VDRL
CBCALT/AST

I'm not even sure what those last 3 are.

Is the EPIK form available online? If all else fails, I might just use it in December and hope that it will satisfy the requirements if I can't get the check done here in Korea.

I had my Epik health check done at a Bogwanso in Korea for 17,000won. They did everything but the Hepititis C check.

You can find the EPIK medical form here. I would probably try to use it since it covers just about everything.

I've had a free health check (everything from blood tests, xrays, urine, bone density...you name it, they do it!) every two years at my uni (offered as a free service to employees). Might the documentation from this do?
