are these available at immigration centers (would assume so but never no)? are there any downloadable online forms? haven't come across this, any relevant info appreciated!

...did ya hear the one about the ambulance workers?

they went out to a call but with traffic and a few pit stops along the way they arrived a bit too late. upon entering the stuffy little cubby-hole of an apartment, they spotted the now deceased ajoshi lying in the prone position. one attendant checked for a pulse, then glanced up at his partner with that 'oh well, too late' look. understanding this look and korean culture, the partner immediately responded with a CYA (cover....) suggestion: "you get the stretcher, i'll plug in the fan"

if all else fails, i got this one with the http://image......yada yada on the bottom of the form.....but hey, white out works...
