Is this the health check form that people who are applying for an E2 visa have to fill out?

The correct answer is the answer that is true for you.

I can tell you from experience that it does not have to be totally clear for you to get approval for a visa. The consequences of not putting something on the form which is picked up later in the health check is not good - with immediate cancellation of visa and the need to pay for your own flight back and in most cases reimbursing your school for the cost of flying you over.

Things that are not in the drugs/alchohol/contagious diseases category and which don't affect your ability to teach are not going to automatically disqualify you from a visa.

Daegu Immigration told me NOT to list a chronic disease I have because it wasn't a threat to public safety. It WAS found on the medical test (as I had known it would be) but the only thing that happened was a lot of doctors got very interested (too interested) in me...

Diabetes and asthma are not a problem on the medical form.

big ol' bump here, but why start a new thread right?

I'm having trouble deciding if there are a couple things worth mentioning on this form. One, I had a couple surgeries a little over a year ago for a kidney stone (not your typical kidney stone). Also, I've seen a doctor a couple times and will have a couple physical therapy sessions for a sternum injury, although it's nothing serious, just some inflammation and pain. Should I mention these? Are they going to require some kind of medical records or doctor's note that might conflict with checking off 'no' to everything?
