I'm in the process of applying for my visa. I'm currently in Canada. My future Korean employer says that I need to send him a medical check-up. I was under the impression that I need to complete a 'Medical Self-Assessment' form for the visa process and when I arrive in Korea, I need to get an actual medical check-up (performed by a doctor) done within 90 days. Am I correct or has the vias process changed?

As far as Korean Immigration is concerned, you are correct.

If the prospective employer requires a full medical check done in Canada before coming to Korea you have the choice of complying or finding a different job.

I just did this (2nd time going to Korea) so I'll try my best.

What forms did you get?

1) Self-evaluation Medical form (to be filled out by you) Those statements are to be attested by you truthfully. HIV, alcohol/drug dependency etc..
2) Medical Evaluation form (to be filled by your doctor) Go to your family doctor since he/she knows about your health history. The doctor will give you shots (Hepatitis, etc.) if needed, check your vision, dental, etc, measure your blood pressure, height, weight etc.

After that, you know already.

I was asking if you need to visit a doctor in your home country. I was under the impression that you didn't need too because you have to visit a doctor in Korea. It would be difficult for me to get an appointment with my old doctor considering the time line I'm on.

You have to fill out the "Self-Assessment" form, and then have a doctor fill out your medical history on a separate form they should've sent you.

When you get to Korea, you go to a hospital or clinic and have a blood test and urine test.

What if you do not have a regular doctor to fill out your medical history? I also thought the self-assessment form required the teacher to fill out the information and you better give the same answers to the doctor in Seoul upon your physical? Confused

FYI, I just looked at the " Self-Assessment form my recruiter gave me to fill out,( Gepik) the form mentions nothing concerning a doctor's information. It asks general questions about your personal health. Confused

The Self-Assessment you fill out (questions about drug/alcohol use, etc.). There should be a second form that says "Medical History" or something, I don't remember, that should've been included in your paperwork. That form needs a doctor's signature. You can probably fill out your immunization dates on your own, but a doctor needs to sign off on it.

When you get shots (Hep, Tetanus, MMR, etc), who did you go to? Have that person sign your sheet, or the doctor you last went to for a check-up. They should have your info as well.

If you didn't get it, you can browse the forums to see if someone else posted it, or email your recruiter and have them send you one.

I don't know if you guys are talking about a special thing for only government jobs or what, but I didn't have to see a US doctor at all. Just the self-evaluation.

Ok so I just checked the immigration website
http://www.hikorea.go.kr/pt/index.html
and it says all you need is a self-assessment. It might just be a GEPIK thing, since I had to get my doctor to fill out my history (mainly immunizations).

Or it could be my school is just really behind and it's part of the old regulations! Mad

I did see a 2005 form showing you needed to see a doctor in the US,but from what I understand the only thing required is a self-assessment form. As far as hep shots,mine were done at a clinic by a nurse not a doctor. The self-assessment issue for Gepik could be cleared up by ttompatz. You have me interested now. Confused
